United Heritage Life Insurance Company – Long Term Disability
As of May 1, 2008 Full time employees are provided with United Heritage Long Term Disability Insurance through United Heritage Life Insurance Company.
Eligibility includes:
- Certificated Staff (Teacher) who work a full-time contract (37.50 hours per week)
- Educational Support Staff who work full-time (40.00 hours per week)
Our plan is a salary protection plan which pays 66 2/3% of your monthly income loss (maximum of $5,000 monthly) after a disability qualifying period of 90 days. Please see certification of insurance for more information. Claims forms are available in the Business Office.

[/one-half]American Fidelity: Short-Term & Long-Term Disability Insurance
Eligibility includes:
- Certificated Staff (Teacher) who work a half-time contract or more (18.75 or more hours per week)
- Educational Support Staff who work 20 hours or more per week
AF Disability Income Insurance
- Can help protect your finances in case of a covered injury or illness
- provides a benefit to help cover costs while you are unable to work
- Pays some of your gross monthly earnings

There are various plans available to you, including both Short-Term Disability or Long-Term Disability Insurance.
Annual enrollment starts in Mid-October through the first week of December, for the Plan Year starting January 1st annually. It is necessary to re-enroll each plan year. The District will notify you of the enrollment schedule by email and place the enrollment schedules in the staff lounges.
