Long Term Disabilities Insurance

As of May 1, 2008, long term disability insurance is provided to full time district employees through United Heritage Insurance Company.

Our plan is a salary protection plan which pays 66 2/3% of your monthly income loss (maximum of $5,000 monthly) after a disability qualifying period of 90 days.  Please see certification of insurance for more information. Claims forms are available in the Business Office.
