Kindergarten Registration

*For all children who will be 5 years of age on or before     September 1st, 2018.

The (3) following documents are required to register your child:

1. Original Birth Certificate (we will not accept a copy of the certificate, nor will we accept a Proof of Live Birth certificate)-if you do not have this, you will have 30 days to obtain it.
2. Current Immunization Records
3. Proof of Residency (i.e Avista Bill, Lease Agreement, rental agreement, etc.)
